Instructions for Downloading and Printing Forms

Our application forms are available in Adobe PDF format. To view and print out the forms, you will need to download and install the free Adobe Acrobat Reader. Instructions for downloading and installing the viewer are included in the Acrobat Reader site.

After installing the reader, access the reader program and open the documents you downloaded. With the reader, you can view and print each document.

To download:

  • Click on the download link to the right.
  • Save the PDF file onto your hard drive. (Remember where you save the file!)
  • Open Adobe Acrobat Reader.
  • Open the PDF file.
  • Print the three pages of forms.
  • Fill out the forms and mail along with the  application fee to:

    The application fee is $300.00 which will cover the cost of handling and mailing costs.

    If you encounter any problems downloading, opening, or printing the forms, please let us know.

    **Note: Upon request, WIA will disclose the following: Service policy Contracts, the number of adoption placements per year for the prior 3 calendar years, the number of placements that remain intact, the number of families who apply to adopt each year, and the number of waiting children eligible for adoption. To obtain this information please call our office.
